LCC Forms and Templates
The LCC enjoins the Local Government Units (LGUs) and Nongovernment Organizations (NGOs) to provide relevant data and information to the Secretariat by accomplishing the monitoring forms (for local literacy councils and community-based literacy programs) contained in this page. The forms and templates for the National Literacy Awards (NLA) are subject for revision, hence the remark ‘for reference only’.
